David Patrick Toth, 37, of Shelbyville, IN, passed away at IU North Hospital in Carmel, IN on June 24, 2021. He was born to Robert and Leisha (Bradshaw) Toth on June 4, 1984, in Evansville, IN.

David is preceded in death by 2 grandfathers, Jerry Bradshaw, and Ronald Toth: and an uncle, Mark Bradshaw.

He is survived by his parents, mother, Leisha Toth of Columbus, IN and father, Robert Toth of Fort Branch, IN; sister, Brandy Campbell of Columbus, IN; brother, Alex Toth of Port Charlotte, FL; maternal grandmother, Beverly Bradshaw of Fort Branch, IN; paternal grandmother, Patricia Toth of Owensville, IN; nieces, nephew, several cousins, aunts and uncles.

A private burial will be held at Clark Cemetery in Owensville, IN. In lieu of flowers, donations can be made to Especially Kidz Health and Rehabilitation, 2325 S. Miller St., Shelbyville, IN 46176.
